As a transitional housing provider we can help to support and make medicine better by encouraging those we serve and in our general community to: (1) once a month review all health care issues; (2) update medication list and keep notes for their next medical visit; (3) check for needed medication refills; (4) review upcoming and needed doctor’s appointments; (5) keep a log of any new symptoms, side effects, or health care concerns; (6) carry information with you at all times for use in emergencies and (7) if possible use "one" pharmacy only and cultivate a pharmacist who knows you.
