anybody ever heard of a medical home? I think it has something to do with your primary care doctor, but I don’t know what it is other than more pie in the sky talk. Does anybody out there have a medical home…and does it really matter?

According to the American Academy of Family Physicians, the term Medical Home , also known as Patient-Centered Medical Home is defined as "an approach to providing comprehensive primary care that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family". This is essentially new nomenclature for a long established model of primary care, whereby the primary care physician is the entry point (home) for most services to be accessed through the healthcare system. At BJC, we believe having a primary care physician (Family Medicine, Internal Medicine, Pediatrician) is important and we encourage everyone to establish a relationship with a primary care physician if you do not have one already.
