| David Knocke 02/24/2010 10:23:10 AM | According to the American Academy of Family Physicians, the term Medical Home , also known as Patient-Centered Medical Home is defined as "an approach to providing comprehensive primary care that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family". This is essentially new nomenclature for a long established model of primary care, whereby the primary care physician is the entry point (home) for most services to be accessed through the healthcare system. At BJC, we believe having a primary care physician (Family Medicine, Internal Medicine, Pediatrician) is important and we encourage everyone to establish a relationship with a primary care physician if you do not have one already.
